About Symbiant SHE Incident Reporting
What does SHE actually stand for?
The word has been innervated throughout the Industry to make things easier to understand and digest. In simple terms, it is Safety, Health and Environment, in the workplace.
Health and Safety is a term that generally covers the legal requirements that fall under the Health and Safety at Work Act etc 1974. The term Health and Safety is generally used to describe Occupational Health and Safety and relates to the prevention of accidents and ill health to employees and those who may be affected by their work.

What is a SHE incident reporter or SHE incident reporting tool and why should I need one?
The term “accident” is also commonly used and can be defined as an unplanned event that interrupts the completion of an activity, and that may (or may not) include injury or property damage. May report to you or to the HSE / LA who may visit or ask for a copy of your investigation report so a good investigation may prevent/shorten a visit Some make a distinction between accident and incident.
They use the term incident to refer to an unexpected event that did not cause injury or damage at that time but had the potential. “Near miss” or “dangerous occurrence” are also terms for an event that could have caused harm but did not. Gather information to notify the enforcement authorities, e.g. HSE/ LA.
Why investigate a workplace incident?
- most importantly, to find out the cause of incidents and to prevent similar incidents in the future
- to fulfil any legal requirements
- to determine the cost of an incident
- to determine compliance with applicable regulations (e.g., occupational health and safety, criminal, etc.)
- to process workers’ compensation claims
This will apply to an inquiry of a minor incident and to the more formal investigation of a serious event. Also, these steps can be used to investigate any situation.
The SHE Software Module, allows you to centrally manage Security, Health and Safety and Environmental incidents from across the business. The module includes all the steps, stages and options so it’s ready to go straight out of the box. As with all the Symbiant modules, you can edit and change, whatever you need to make it a perfect fit for your needs.
The forms are dynamic, so they ask questions and capture data, based on the type of incident that is being reported. The module comes pre-loaded with options that relate to the type of incident, to ensure you capture the information you need.
Once the incident has been logged, notification emails can be sent to make people aware of the issue. The built-in investigation form let you capture and log the root cause of the incident, to help prevent the event from happening again. It also allows you to log and capture all of the required steps, to ensure the investigation has been completed correctly.
If required the SHE Software enables you to create actions and assign them to people with an action due date. These assignees can keep you updated with their progress so you can track them to completion.
Documents and files can be attached to the incident, with clear details about the files, together with who attached them and when.
You can edit the guidance tab to give your users, your own specific information and instructions, not only does this improve the quality of the reported data, it reduces the need for users to seek help from colleagues.
The clean interface and graphical filters organise the incidents by type and let you see the current status and all the relevant information, at a glance. The module also includes detailed reporting and automated notices.
